Subject: Extension of my holiday

Respected Mr. Daniels,

This letter is in regards to the leave that I had applied for from 1^st of May till 10^th of May. I would like to inform you that I will require an extension of my holidays.

There are few personal matters that I have to take care off. My mother is not keeping well and she has to undergo a surgery after which I will have to spend few days with her. I would require an extension of five days that is till 15^th of May 2012.

I request you to look into matter and allow me to extend my holidays till 15^th of May. I would be grateful if you accept my request.
