Subject: Claim for deduction from my savings account

Respected Mr. Lee,

This is to inform you that I would like to claim for the deduction made from the savings account number 9393-39393-3939 that I hold in your bank. The amount was deducted as annual fees of the credit card issued by your bank.

I was checking my account statements day before when I realized that an amount of $100 has been deducted from my bank account for the credit card issued by your bank. I would like to inform you that I had never applied for any credit card from your bank. Above that I was also never informed about the deduction from my account.

I would like you to credit the amount that was deducted. I also request you to please cancel the credit card you have issued as I don’t need it.
