Subject: Cancellation of Credit Card

Dear Mr. Smith,

I would like to request you to cancel my credit card with effect from 1st March 2012. My Credit Card in the name of “Paul Anderson” has number: 93837-383873-3783983. I hold a credit card account with you since the last 5 years and am quite satisfied with the services your bank has been providing. I have hardly ever faced any problem while using your credit card and appreciate your bank officials for the same.

However, I would like to inform you that I would no longer be able to continue this account and the credit card as I am relocating to a different place. I would therefore, request you to cancel my credit card as well as my account.

Please accept this letter as a formal notice for cancellation of my credit card and let me know if any other formalities need to be completed for the same.
