Subject: Request for Cancellation of Insurance Policy

Dear Mr. Smith,

I am writing this letter to request you to cancel my insurance policy number ATS/9398/3839 with effect from 20th Feb 2012. At the time of taking up this policy I was informed that in case I withdraw it in between, I would be entitled to the unused amount of the policy. I would therefore, request you to provide me the unused portion.

Please accept this as a formal request for cancellation of my insurance policy. The other details of the policy have been enclosed with this letter. Kindly let me know if any other formalities need to be done. You can contact me on my number 098-98877990

I would appreciate a written confirmation once the cancellation request has been processed.
