Dear Sir,
I would like to let you know of my decision to terminate the insurance policy with the number 237690 which was active from the 23rd of January, 2010. I would like to cancel the policy without much delay and before the last day of May, 2010.
The premiums have been accurately paid so far and I believe that unused amounts can be obtained back on the premiums paid so far. I would like to receive the unpaid amounts on the mailing address given with this letter.
I would also like a written document stating the closing of this insurance policy as a confirmation. I hope to collect this letter also within a period of thirty days of receiving this letter for cancellation of the existing policy with your service.
