Dear Dr. Wilson,

I have been taken ill three weeks ago and have been taken to a hospital for proper diagnosis and treatment. We have incurred considerable expenses which we have taken from our meager savings. We were not hesitant to use it as we a have Medicare coverage to provide reimbursement.

However, when we went to the local Medicare office, we were unable to get the Medicare coverage which we have the right to have as provided on the Medicare coverage. We could not get much headway with the local staff so we are writing to you for assistance.

We need my coverage so much and I hope you will not disregard my appeal. I have my contact number enclosed with this letter and on the form I signed up when I took a Medicare.
