With the help of this letter I would like to inform you that I am not going to be your physician any longer from next month onwards. There are some personal problems that I need to sort out so I will not be able to treat you due to this.

There are many other physicians in the city that you can consider for your treatment. If you want then I can suggest you some of finest physicians in the city who can treat you best. I hope that you have been happy with my services as physician.

I would like to request you to clear your all dues with me and expect early reply to this letter from your side.
