Our record shows that you have been given disciplinary action for several instance of unauthorized leave of absences. The first offense was committed on June 15, 2011 where you were given a verbal warning. A second offense followed on July 1, 2011 and a written warning was issued to you. On July 8, 2011, a third offense was committed and a final written warning was issued together with a one-on-one coaching from your immediate supervisor reminding you that a fourth offense will mean a dismissal. Despite of this, you incurred an unauthorized leave last September 15, 2011, being your fourth offense.
Your repeated neglect to comply with company rules and procedures prompted us to terminate your employment. You may claim your final pay from the Payroll Supervisor upon surrender of all company liabilities.
We shall extend to you our policy of disclosing only your Job Title and period of employment with us for prospective organizations seeking professional references in your name.
